Case files
Each case file starts with an event: an AI system did something that does not fit the picture of a machine doing what it is told. It then shows how named people read that event, in their own words, and what evidence would settle the question.
The site takes no position. Every quote is checked word for word against the page it came from. A case file is published only if the company that ran the system, or the party it affected, has given its own account of the event, and at least two named people read the event differently. An honorable mention misses one of those two conditions and says which. Software drafts and checks each case file and publishes it without human review.
The record holds only what breaks the picture of a machine doing what it is told. What AI systems do as expected never appears here, whether or not anything lies behind it.
